Minority Health and Health Disparities

Differences in the quality and availability of health care are problems which significantly affect the health of minorities, and affect likelihood of positive care outcomes. Join Dr. Kenworth Holness, of the Carl Vinson VA Medical Center in Dublin, GA, as he discusses what health care disparities are and who is affected, why disparities exist, and what steps are being taken by clinical care providers to reduce and eliminate health care disparities. Dr. Holness will also discuss specific tools individual providers can use in their practices to address these barriers to care.

The federal government has adopted the patient centered medical home model within the Veterans Administration. Across the country, private and community based practices are looking for guidance on how they can do the same. This is a new concept with goals to include increased patient access, care coordination, health information technology and payment reform. Ultimately successful integration of the PCMH should achieve the goal of better care, improved health outcomes, and lower costs.

This presentation will outline the content area of the 7 Self-Care Behaviors along with methods for effective education in promoting behavior change.
